We offer concierge primary care services, and treatment for common health conditions for patients 13 and up.
Concierge intermediate membership plan
12 visits annually with comprehensive annual exam, in-office & telehealth appointments, trade in for at-home sick visits from your provider, annual vaccination, vitamin injections, annual nutrition counseling, dedicated VIP Concierge Patient Coordinator for assistance with diagnostics and referrals, after-hours contact with your provider between 5-7 pm.
Concierge complex memerbship plan
